Application Checklist

When completing your child’s Hope Scholarship application in the online portal, you will need the following information/documents:

  • Your child’s birth certificate  issued by a state – not hospital.
  • Proof of West Virginia residency Demonstrated by one of the following criteria that matches the physical address used on the application:


    Current tax records;  •  Proof of West Virginia home ownership, such as a mortgage document or homeowner’s insurance document;  •  A valid West Virginia vehicle registration card;  •  A valid West Virginia driver’s license or issued identification when presented with a second form of accepted proof of residency;  •  Current proof of public assistance through the West Virginia Department of Health and Human Resources; or  •  Two West Virginia utility bills no more than 60 days old from two different companies, not including termination notices. The following are considered utilities: water, gas, electric, telephone, cellular phone, garbage, internet and cable.

  • Your child’s grade level for the year for which you are applying for the scholarship
  • Your child’s WVEIS ID# assigned by a West Virginia public school. This can be found on your child’s report card or school lunch bill. If you are not sure of your child’s WVEIS ID# or you do not believe they already have one, please complete the WVEIS ID# Request Form to request one from your local county board of education. The completed form should be emailed to your county of residence using the email address from the county board of education email list found here.
  • A Notice of Intent Form

    All Hope Scholarship participants must submit a Notice of Intent (NOI) form to the county board of education of their county of residence. You can find the contact information for the county superintendent at the following: WV School Directory (state.wv.us) and the email address to submit the NOI at the following: Hope County Board Emails.pdf (hopescholarshipwv.com)

    • The Hope Scholarship NOI for students in grades K-12 should be submitted to the county board of education AFTER the student’s Hope Scholarship application reflects a status of “Eligible” in the Hope Scholarship online portal. Please be sure to indicate the correct school year for when the student will begin participating in the Hope Scholarship Program on the student’s NOI, as submission of the NOI also serves as a notice to withdrawal the student from public school at the appropriate time if the student is actively enrolled in public school.

    Please note that the Notice of Intent to Participate in the Hope Scholarship Program is different from the traditional Notice of Intent to Homeschool. It is important to utilize the correct Notice of Intent language depending on which educational choice you are making for your child.
